Dear Prospective Patient

 

Thank you for expressing interest in the services of the UCLA Predoctoral (dental student) Clinic. 

 

For your appointment please bring and follow the instructions below:

 

  • Complete the Heath History Questionnaire online.
  • Bring a list of your current medications and dosages.
  • Bring a current non-expired government issued photo ID (drivers license, passport, military card, border crossing card or UCLA student issued ID).
  • If applicable, bring your Medi-cal card or Delta Dental identification card.
  • Arrive 30 minutes early for your first appointment, and 15 minutes early for subsequent appointments.  Please plan sufficient time to allow for the traffic congestion and parking.
  • Please remember we require a 48 hour cancellation notice.
 

During your screening appointment, a licensed dentist will perform a brief oral screening to determine if your needs are consistent with the teaching guidelines necessary for our predoctoral students.  If you are accepted as a patient we will register you by creating an electronic patient record and you will have all necessary radiographs (x-rays) taken if you do not have radiographs that are current (taken in the last 12 months).

 

If accepted as a patient, you will have a set of radiographs taken directly after the screening appointment. The cost for radiographs is $54. Your next appointment will be three to four hours long, and there will be a fee of $49. This visit includes a comprehensive examination, all consultations and an itemzized treatment plan.

 

If you are bringing your own radiographs they must be on a CD or flash drive.  UCLA does not have the ability to add traditional film (non-digital) radiographs to your electronic patient record.  The examining dentist will review your existing radiographs to determine if the quality is acceptable.  There is a cost of $28 to add radiographs to your record. 

 

Our dental clinic operates very differently from a private practice as this is an educational institution.  You will, however, receive professional care from the student dentists who are closely supervised by our highly experienced faculty during your three hour appointments.  If you are accepted as a patient, it may take up to seven working days for you to be assigned to a student dentist.  Once you are assigned to a student dentist, you will be contacted to schedule your next appointment.
